New Patient Health History Form – Required
Please complete this form as it lets us know the history and current state of your health. Let us know what questions, concerns, and goals you have regarding your eye health or vision on the form. It is vital for us to get a good patient history. Please note medications are a must to understand your eye health.
New patients will receive a link via text or email that they will need to fill out prior to their exam. It is vital to get an accurate health history including any and all medications.

- Fax your printed and completed form(s) to our office or bring them with you to your appointment.
